Patients of Northwell who logged into the FollowMyHealth patient portal or booked an appointment on Northwell’s website between January 1, 2020, and December 31, 2023, or patients who were patients of Northwell between January 1, 2020, and July 25, 2024, and are not included in the first subclass.
